What we may need
For a request, useful information can include your name and contact information, the activity or object involved, measurements, photos of the relevant object or setup, and a description of the barrier and desired result.
What not to send
Please do not send medical records, Social Security numbers, insurance information, financial account information, passwords, or other information that is not needed for the project. A diagnosis is usually unnecessary.
Email and photographs
During this early launch, request and volunteer forms open an email addressed to accessmadeiowa@gmail.com. If you include photographs, review them for addresses, paperwork, faces or other details you do not intend to share before sending.
Project sharing
We will not treat submission of a request as permission to publicly identify you. If a project is useful as an example, we may ask for permission to share it, or share only de-identified details when appropriate.
Children and other people
If a request concerns a child or another person who cannot submit it themselves, provide only information you are authorized to share. We may ask to coordinate with a parent, guardian, caregiver, teacher, therapist or the recipient when appropriate.
